Humana Launches Program Regarding Social Determinants Of Health
The health company Humana has always supported its clients beyond healthcare. Recently they announced a new way of doing this through a value-based care payment initiative. Through this new means, the company will address nonmedical heal risk that might be affecting the members negatively. The Social Determinants of Health Value-Based Program will aim to improve those outcomes using a holistic approach.
Social Determinants Of Health
Social determinants of health can vary, but all pose a risk to clients’ health. These factors can include food insecurity (inconsistent access to nutritious food), social isolation (lack of interaction with other people), loneliness (feeling of being alone), and housing instability (lack of stable housing). The current model will also provide tools to help combat the social determinants of health to all members who need help. The three components of the program circle around patient screenings; documentation of assessment findings; and connecting the patient to appropriate resources.

We’re better able to support clinicians as they broaden the scope of patient care to improve health outcomes.”.
Programs Backed By Humana Providers
The program is being backed between Humana and its providers. One provider Ochsner Health is Louisiana’s largest non-profit, academic health care system. The plan is to continue moving forward offering the agreements to other providers as it seems to be widely received. A recent report on Humanas Value-based Care, done last November showed that clinicians in the arrangements enjoy more engagement with Humana MA members compared to providers in non-value based contracts.
